SERVICES AND PROGRAMS

Skilled Nursing Services: Our facility is staffed around the clock with well trained RN's and LPN's who provide many different skilled procedures such as Medication Administration, Enteral Nutrition, IV Therapy, Respiratory Care, Pain Management and Diabetes Management and Teaching.

Sub-acute Care Program: Bridging the gap between hospitalization and home, our Sub-acute Care Program provides patients with the opportunity to increase independence and complete their recuperation in our 41 bed separate and newer state of the art sub-acute unit established in 1996. Our interdisciplinary approach to care creates a clinical pathway to optimize restoration of function and positive outcomes. Sub-acute services include complex medical care, oncology, wound care/management and cognitive retraining.

Short-term Rehabilitation Services: Provided by our team of highly skilled therapists, Bear Hill's short-term rehab services include physical, occupational and speech therapy programs up to 7 days per week designed to meet the individual needs of each patient. Customized programs include: Cardiac, Pulmonary, Stroke, Post-operative Rehab and Orthopedics. Comprehensive family/patient teaching and home evaluations are provided to maximize successful reintegration into the community.

Long-term Care Services: The foundation of the Bear Hill organization is our long standing commitment to provide the very best of long term care and supportive services to our Geriatric community. Our endeavor is to restore and maintain the physical and psycho-social well-being of our residents in a caring, upbeat and dignified setting. We strive each day to make the facility a home and, above all, to honor and respect those entrusted to us.
Hospice Program: We offer specialized comfort and palliative care for those individuals at the end stages of a terminal illness or life. Services include: pain management, support for the family, pastoral counseling and spiritual support. The program provides the patient with control, comfort, choice and dignity while respecting the patient's and the family's right to make end of life decisions.
