Our expert subacute rehab team of therapists closely interfaces with our physiatrist to develop a plan of care that will help you achieve your optimal level of function and get you home as quickly as possible. To help you successfully integrate back into the community, we implement a safe and efficient discharge plan and coordinate your post-discharge care.
Our medical team includes our medical director, attending physicians, full-time nurse practitioners, and a pulmonologist, cardiologist, and wound doctor, who are all committed to reducing hospital readmissions and helping our residents regain their mobility and independence as quickly as possible. The modalities we use include: Ultrasound, E-Stim, Diathermy , Nu Step Machine, Omni Cycle Machine, Dynamic Gait Trainer and Stand Assist Trainer.
